Looks like it was a bad capping station. One of the hoses for the waste ink was completely clogged, which fouled the capping station and caused cross-contamination. Replaced it with fuel line for model airplanes - back up and running. Thanks!!

Thanks for the suggestions I checked all the cables and they were fine, so I updated the firmware. That seems to have taken care of it - it hasn't done it since! Thanks to everyone for taking the time to help!
