Gerber AutoCarve did everything we ever needed to do.

Go to the local hardware store and buy a de-burring tool to finish the edges by hand after routing. We've been doing that for years and it works like a charm.

We always used grommets for the added reinforcement and neat, finished look.

IMHO, if you've used different foils on different vinyl and the same thing happens in the same place on your designs, then there has to be an issue with your art. Check your overprints because there may be something in the other layers that's knocking out your flood coat.
