Know nothing about a mutoh, but that last one looks like too much heat on any machine...... or it's hitting something and getting crumpled up.

I print a lot of window perf on my Mutoh 1624. I've never seen this happen. I agree that it could be a bad roll. Is it coming off the roll like that before printing or just after?

Strange. I'd want to see what the material looked like before you print it.

Some people have suggested less heat; that sounds like a good idea. But it might just be a bad roll; how long have you had it?
 
I've never seen a valuejet get hot enough to do something like that. I suppose it's possible so you could try lowering the heat, but my guess is bad or old material.
