The190evo issues

Hey guys.
Had this problem on two different occasions. Where a latex printed graphic with hexis 190evo been bubbling up on flat surfaces. See pics. What could be the problem here?
Everything by the book. Prep. Print. Clean. And everything. The tac is really really low.

Hope you can give me some ideas.
