Need Help VJ 1614 - Yellow Only

Ink Spot Printing
I have a mutoh vj 1614 and it only prints yellow.... Any thoughts? I have a new printhead, new dampers, and when I run a little charge it pulls lots of ink. Could this be a damaged head or fuse error?
