artwork or design agreement form

I am trying to get some proper wording for a design agreement and or guideline for the customer to sign before we start designing.

A form of some sort that does not only requires a artwork charge, but also saves our butt if someone else copies our work.... What I am trying to ask is do you have any forms I may get an idea from by chance?

best bet, go to your attorney and have them write something up

here is my art disclaimer, attorney FREE! I have art copied but not stolen. Everyone gets low res jpg unless you decided to use me.

Here's what I've been using for 20 years.
(my brother is an attorney and he reviewed it for me)
Most people do not want to sign it...I use this as sort of a gauge of their character.
It's merely a proposal.
I know $25 seems cheap for a washing fee but it usually insures that I am presented with a clean vehicle.
