Best process for project sign off?

I am currently trying to design a way that we can get some sort of written approval from our clients when the artwork has been approved. The current way is sending a pdf proof and getting a reply email saying ok or good to go, nothing concrete. I would like to send another pdf that they would fill out and send back to us. I have made a pdf form they they can fill out its just how to get that information back. I can have them print it off and fax but not everyone has a fax machine or have them drop it off but not everyone has time to do that.So my question is what does everyone else use or do they know of another method that we can have on file a proper sign off sheet.

I meet face to face, so any manners can be dealt with and we both are on the same page approved, deposit when needed. course I only do a few jobs a week so more time.

Do you set that up for every client proof or is the process automated? The way it looks you have to upload an HTML page every time you want to send a proof and that is after you configure the images, set a unique login for each person and then send them a link?
