can not fix print quality on Mutoh VJ-1204

After I have clean the print head many times and then washed them I still get these results. I have even cleaned the flushing box sponge (see attached files) Can anyone figure out what is going on?
Sometimes the spots follow the roller patterns.

either laying down too much ink (profile related), wrong media (solvent vs waterbased), or contaminated media

those are my first thoughts.

Thanks everyone. I changed the media and the print quality is back to normal.
