When you setup to print your output values chart in your RIP start with 0/100/100/0. Then choose the best output value that matches your sample. Then create a named spot color so that the RIP will choose that output value or use the replace color function in your RIP.
It's possible that the color you are trying to match cannot be reproduced by your printer (is outside its color gamut). In this case, knowing the Pantone number will still not allow you to exactly match the color.
