It's most likely cross contamination. Usually it's caused by a failing maintenance station or damper but it can also be internal damage to the head. I'd start with changing the cap top and possibly the dampers especially if they are older than 2 years. You might as we ll flush the pump tubes while changing the cap top to make sure they aren't clogged.
