Need Help Font Help and Effect help

Hey all,
Can anyone help me out with this font?
Also how is the 3d type look effect accomplished with Illustrator. Client needs as close to this as possible.

All of the effects added to the type look to me like they were added/drawn by hand in Photoshop or some other type of raster image editing software.

Kind of what I was thinking. Unfortunately Me and Photoshop do not see eye to eye. Will have to come close with something in illustrator........
