I have some good news and some bad news.

Good news: The name of the font is Koch Antiqua Extra Bold that has been stepped on a good bit.

Bad news: The font is manufactured by Alphabets, Inc. and sold through an exclusive license with Linotype ... but only in the regular weight. The extra bold weight is shown on the home page of Alphabets, Inc. but their site appears to be only a home page with no links to any fonts.
