How to create table label?

Hello,
We have had a request to create a table label similar to the image attached.
Dimensions are 6"x4" - but these are flexible. Has anyone created something similar?
We have not been able to find something similar and unlabeled online yet.

Any info on purchasing or creating is welcomed.

I've used C&C Engraving out of Arizona to make these for us. If you're needing something engraved and not imprinted.

In Corel we made a template to print to heavy card stock. Create a list in Excel. Plug in the names and the cards are filled in for you using Print Merge. Hand cut, score and bend. Made thousands of these over the years.

I've always just cut up styrene or acrylic ( depending on customer budget and use) and bent with my tabletop strip heater or a heat gun over edge of table for fold. Once you've done the first you can use it for a form.

We do these as well. Table tents in custom sizes out of styrene or PVC heat bent.
