Would anyone mind sharing the written terms you require your client to agree to when they purchase a logo design from you? I'm doing more design work lately and I want to prepare a formal document that spells out the rights and responsibilities of each party.

I actually dont prepare any written terms for a client when I design a logo for them.
When I get it done, I prepare two cds with all the filetypes needed on them, give them to the client and we are done. They own all rights to the logo at that point and can use it as they see fit. That is, after they pay the agreed upon fee for the design.
