I got the form off the pa state website, now my question is do i have to fill out the form and send it in to the state, or do i just fill it out and give it to my supplier? I just do vinyl(reselling the vinyl, application tape), so im assuming i should be taxed on stuff like cd's, squeege's ect correct?

You fill it out and give it to your supplier, and any one that you sell to that claims tax exempt you have them fill one out!
