Phantom of the "Printing"

Have anyone seen or heard of these images popping up on projects? It happen in early April and just today. Any remedies?

What print method & substrate is this?

Both were printed on vinyl MPI3300 Gloss Perm CCK 60. Now that has me thinking. I don't get it on any other substrates. Thanks JBurton

Lifting? We were looking at both of them up close. Seems that there is some sort of similar phantom patterns. Going to check files and post my analysis

Both files were created in Illustrator and no mask found

Lifting, I think he means like tunneling, like is that section of vinyl actually separated from the backer?But I think he was imagining it was clear film...
