Plexi-glass office signs

We have a client looking to update their door sign. The client would like plexi-glass substrate. Who is a good supplier for these.
I have attached an example of what we are in need of.

that looks straight forward,

1st surface vinyl graphics, with a 2nd surface full coverage (guessing paint, might be vinyl)

contact your substrate supplier, have them cut the material, JDS also does this, then apply the information
I'm guessing that is probably P99 non glare

I`d cut the plexi on a cnc machine then ad graphics
