"POP" goes the head. Is my slider or main board killing my heads?

So last night I as trying to get the 2nd printer hooked up with onyx. While tring to install the drivers the printer made a loud pop noise and the middle head 06'd. It was test printing fine until that moment.
Its funny because 2 other heads showed 06 in the middle position.

What could be making the heads fry?

Most likely a short. There's the possibility that the board is fried, but that should've given you another warning, at least before it blew up the head.

Check to make sure all your connections are in square, and not at an angle that may cross over more than one connection, and that your cables aren't damaged in any way. All it takes is one contact.

Is this a new machine or a refurbished one?

The heads should be protected by a fuse!
