Recommended designers & logo design cost...

(Seems like this would be a nice sticky unless recommending a non-merchant designer is against forum policy...)

I did a search for this topic, but the keywords are used in virtually every thread title and post, so the results were a bit overwhelming.

Who are the recommended "go-to" guys & gals on the site when it comes to logo design and how are rates/fees typically charged?
