On a couple occasions, everything looks fine when the graphic file was created and saved or when supplied by the customer.
When the file is opened up for ripping/outputting, a double cut line appears.
Any ideas as to how this happens and what I can do to rectify the issue?

    Many thanks...

Sounds like the stroke is converted to outlines giving you two "strokes"

    Or....

your elements were "combined" prior to
importing OR the original artwork was created in Illustrator (sometimes)

I thought so too but in Illustrator there is no indication that it was converted.
It doesn't appear until I'm ready to send the job to the printer.

In this particular case, I created the images in Illustrator.
99% of the time I have no issues. The odd time I will have the double cut thing happen.

I use Illustrator almost exclusively and never have that issue. I would check the file again and perhaps delete and recreate the cutline to be sure there really is only one line

The following message sometimes appears when this double cut line thing happens...

"mixing process and spot color swatches may result in...."

Not sure if that could reflect this issue.
