I was just wondering what cause this? The second picture is the worst one. The first picture is after I cleaned it. It still there but not as bad. Is the head going out?

DIY Printer Fixing Guide
Usually when the test print "bends" like that there is an electrical issue (signal timing) or you had a really bad head crash and the nozzles are physically damaged. Try re-seating the head cables and check for any damage or ink debris on the leads. Otherwise you could have a bad head, cable or even the main board. Also check the encoder strip for ink, grease or any other debris that got on there as that can also affect the test print in that way.
