trouble with jv3 new head id

I just installed a new cyan head. Mechanically, all went well.
tried to put printer into service mode to enter new head id by holding function and remote keys while printer was booting. The "input head #" screen never came up, the printer just started as normal to the "roll-sheet" screen.
Did a test print, m y and k printed fine as in attached photo, cyan only printed 1 side. I used syringe to pull ink through head, swapped damper, and ran clean cycle, still only 1 side of test print. Then created a cyan rectangle in flexi to test print. Cyan head printed a cloudy print even outside rectangle. Other attached file.
Any help would be very appreciated.

You either have a damaged head (either by inserting the flush too hard or received that way) or the slider/main board is damaged (in which case you need to get that repaired or replaced.
