workable PDF

What software can i bring a PDF in and make into a workable PDF so that you can type in the answers and fill in the boxes? Want to make this pdf able to be opened from a link on a website and be able to fill it out. This is a cut and paste of it. I have the pdf file.

I don't acrobat Professional. I just have reader. Will professional do it?

thanks tres. I hope to have some good news today for the new machines. They finally got all the i's dotted and t's crossed. They had a problem with my new address had to send in all kinds of proof that the building exsited. LOL
