I would agree, I think this does look like some sort of processing glitch. Maybe try a different file format or re-rip your file.
Hopefully someone else on here can offer some more input.
Check this thread out here. Sounds like it might be a similar problem that Graphix45 was having. He cleaned the encoder and seamed to fix this problem.

I would do a "Calibration" on the specific media your trying to cut. Make sure the setting looks correct and there is no overlap or gap between the two squares that get printed. If this setting is off it could cause this.

It's possible the captop is bad and not circulating the black ink properly and you got some ink built up on the head. I know Roland wouldn't advise you to do this but Ive done it plenty of times. You could take your cleaning swab, soak it with fluid, and carefully hold it on the black head top...

Your black head test print looks pretty bad... When is the last time you did a manual clean?
I would try doing a manual clean, replace your wipers if they need it, then run a medium clean and see if any of that clears up. Your magenta is misfiring in a couple spots as well.
