I've had similar issues resulting from:
-Print feed calibration was off
-or the lights in the studio are way to bright on gloss materials and I put something over the sensor as it reads the REG marks to shadow the red sensor.
-OR and this is a big one, this happens alot, if there is dust anywhere on that registration mark it CAN mess with the red eye sensor
