Does anyone know a company that could produce blanks like in the photo?

Grimco sells a hot bending machine that will do excatly that and it cost in the $300.00 range.

There are custom plastic packaging companies that should be able to produce these for you, think of the type of places that produce p.o.p. displays. Not necessarily sign companies.
