looks custom.
1/8"-1/4" photopolymer header (painted & stamped letters), unless the letters are a cast-color all the way through then it is most likely rasterbraille header instead.
1/8' white acrylic backer, & 1/8" clear acrylic "lens" with 1/16" spacers top/bottom & routed "thumb hole", & first-surface vinyl graphics to hide the hardware.
anyone in your area that does ADA signage, like an engraving/trophy/sign shop, would have ability to do these.
