supplier for custom acrylic office signs

I can't find a local plastic fabricator willing to take this on...see the attached pic. The acrylic portion is v-carved second surface and has beveled edge across the entire perimeter including the curve. Does anyone know of any suppliers capable of doing these (and willing to do them in small quantities). Thanks in advance for your suggestions.

I should mention that we are only looking for the acrylic pieces...not the entire sign.
