Well, I went to the mall earlier today and looked the T shirt booth, he only had a variety of shirts pre-printed then he had a plotter with some plot & press vinyl, a Canon inkjet printer/scanner with some paper T shirt transfer material and a heat press. I'm sure he plotted and pressed transfer vinyl for name customization and scanned, printed to paper transfer and pressed photos that people would bring.
