One of my local plastic suppliers has 1/2" solid PVC sheet, its not foamed in the middle, weighs a ton and machines beautifully, but if I remember correctly, it's no cheaper than 1/2" thick acrylic so there is no real reason to use it.

Could you process your decals in "sheets" like 11x17 sheets with as many die cut decals on them as you can fit, then perf cut the sheets out on your plotter? We do this for some clients, saves a.bunch of time cutting and packing decals individually, and still has a nice organized final product.
We buy something called "washi tape" from a tape distributor here, it is sold to body shops, sticks really well and doesn't leave any residue. Only costs $2.99 a roll as well.
We are able to do short runs on our epson s80600, we use our summa f1612 to kiss cut the decals then cut them into long strips and splice them together and roll them up. You can make batches of 1000 or 2000 pretty easily.
Note if the client is going to be applying them by machine my method...
We outsource a ton of bronze plaques to Gemini, at any given time I have 5-10 projects with them, I do the design and send it off, and usually they will blind ship to my client.
You mentioned it happened 3 times for this file, did the issue happen at the exact same point in the graphic on each? Could be a corrupt file, or the file was corrupted in the rip.

The epson inks seem to scratch fairly easily. the Latex machines have much better scratch resistance but at a lower quality finished product.
You can get cheap laminate with no UV properties for around $80/roll, it wouldn't add too much to the cost, probally less expensive then checking for...
