    Ground the printer frame itself well (get it off carpet if possible) and go on amazon.com and find a couple cheap anti-static things (I hear the tinsel-looking ones are particularly effective) and try them out. No idea why/how exactly they work, I haven't really had static issues that I can recall.

    Or are you maxed out on height? I would suggest putting a box around all the artwork, but your 4-corner boxes seem to show there is another issue. I'm betting somewhere the artwork has an error in it, and that is confusing the machine. I assume there is a stray point somewhere causing the issue.

    sounds like electrical. possibly the ribbon cable or slider board. All the other test prints are confusing me, can you do another print and have only it in the picture and take a better photo? Why was the head replaced?

    IIRC I bought a genuine Roland part encoder strip for right around $120 back in the day... I think we bought all of our parts from Nazdar. Shouldn't be anywhere near the price you're looking at.

    You can also flip the encoder strip upside down and use that portion if the top is damaged or stained beyond repair. I think you have to cut a new little hole with an exacto but it should be easy enough, it's been 6 or so years since I've had to do this.
