Yes, it could be a heater or thermocouple issue.
But I would expect that on every print, not just the 'second'.
If you are connected directly, you should be using a cross over cable.
You may want to export all your profiles and reset your "Preferences" to default.
Sounds like a miscommunication from the RIP.

The sensor for media is on the carriage, check the connections and position of the sensor, it can be knocked out of place.

Could have an air leak.
Or just an air bubble if the cart was emptyl.
Have you tried a series of 'cleanings' while watching the line to see if it fills?
There should be profiles installed when you install the printer.

Profiles for 1204, 1304 and 1604 should all work fine.
