have you done anything to the machine recently, like replace dampers/printheads? my guess would be your issue lying somewhere between the slider board and the printhead, most likely either the ribbon cable or the connection on either end of it.

cutcontour needs to be set as a fill, not a stroke. it will recognize strokes as double lines. onyx may just be having issues with the stroke/font-outlining step. convert text to outlines, with cutcontour spot color fill.
