    One way to rule out the ONYX and force the Mimaki tech to look a little deeper would be to find a way to check out the RIP'd file that gets uploaded to the printer. If it doesn't show the mystery gaps then it's definitely the printer. Onyx must have a way to find and check this file.

    Try an overnight head wash. Also you might try using the syringe method to draw ink through the head. In the end if it's not firing due to damage, then replacement is your only option.

    Regular maintenance which includes the dampers and capping station is really dependent on usage. As for material settings, your best bet is to check with the material manufacturer. Many have downloadable spec sheets with just that information.
