i think all of the options have some merit. perhaps give the customer 2 or 3 options. they may like one window in one option and a different window from another option. they can mix and match if they want but nice to give them a couple options

probably about $150-200 in material so would charge around $400-450 for the sign and $150-200 for install so you are not way off but todays market is inflated due to material and fuel costs so get as much as you can get with out pissing anyone off
i would tell her to have the customers contact you. you don't just give out customer files/logos with out consent of said customer. if they did pay you an art design fee you really should give them a copy of the file. that's what we do.
