I re-read your post but no where does it state exactly what make and model your machine is.
So what make and model of machine do you have?
I believe that earlier DX5 print heads are interchangeable with other machines but the later ones are coded and...
This is a number of easily fixed problems or one expensive one.
1. Head cable swap with new OEM ones.
2. Ink contaminant on the ribbon cable or on the print head circuit and cable socket remove print head and clean thouroughly.
3. Slider Board, Swap cables to check that your problem remains...

I think you will find the card readers inside the machine are damaged with sliding in and out cartridges.
I have had to change them all twice on my 1604!

I would hope a print head would last longer than 14 months! Unless it runs 24-7.
Always a good idea to change the cable when fitting a new print head.

That looks to me as if the head cable is failing or the print head is failing or maybe both.

I am not so sure whether the extra weight of this device may cause extra wear on carriage bearing, motors and pulleys.
Also I am not too sure that fitting this device will maybe cause air leaks in the lines as the one sure way to make sure of no leaks is not to have any joins...
The fuse on the main board is very very small and a pain to replace it may have blown and is only showing it is working due to a return circuit.
But realise that the fuse has blown for a reason and it is there to protect the main board so something is not right.
You probably have a faulty head...
I have noticed a fault which I have no idea whether it is a RIP problem or a machine issue.
These GB stickers were stacked up to my printer with a 0.03mm gap.
Yet when it prints them out the gap can vary up to about 1mm.
The issue is if you reset the job the gap is always spaced out differently...
I have been given a job something like what I have attached.
This was created by applying translucent vinyls over the inside of the panels.
What I am trying to do is print a pattern something similar to this and apply it is a single print.
Question is can I print onto clear with my Mutoh...
Check and clean the head and ramps either side.
Clean or replace the wiper
Clean or replace the cap.
If this doesn't fix the problem clean the encoder strip with denatured alcohol.

Sometimes it can stay in the lines and sometimes it can go all the way back to the cartridge.
Also check that the cartridge has ink in it (I know this sounds like obvious but you would not believe it).
Also check the ink in the cartridge is actually exiting into the line.
If you run the pump...
Possible issues are:-
Dampers
or
Blocked pump
or
Head manifold (Can crack and cause an air leak)* This is the most likely one.
or even
Leaking lines (possible on an old JV3)

We have had a 1604 from new (2007) and I cannot fault it (mind you it is is kept spotless).
It has had one print head and has run on bulk inks for the last 2 years.
I actually have looked at buying a second 1604 but have been put of purchasing someone elses...
Our biggest issue is the weather here in Cumbria, England.
Sadly our issue is sunlight (more like the lack of it!).
The weather here changes from wet, very wet to deluge!
Also doesn't help being near the sea which causes issues.
