Hmm, I prefer the second one. The bullets are positioned correctly and it doesn't have an unfinished/run-on background that should be avoided with logos. The cut banner on each side looks a bit odd my opinion.

Bad foil or bad material causes that. Doesn't mean the entire roll of vinyl or the foil was bad but that's most likely your problem.

speckled ='s halftone, fisheyes on bad material or wrong profile/heat setting. Post a pic.

Are your colors set to spot or cmyk? When a head goes bad you usually lose a pixel and will get a very thin white line through the print.

Does he have those "cute eyelashes" over the headlights now?...
