You've already got a start on design.

I use Stahls ThermoFilm on dye sub jerseys (mainly motocross) and have never had a problem with opaqueness or washing. Did you ask the customer how they washed/dried it? Are you sure you used the proper settings when you pressed it? Did it look good when it left your shop?

I'd never ask a customer to pay $325 on an 18in x 18in decal that had a 2 year life expectancy. That's outrageous.

With vector artwork, I'd charge between $45-65.

Just had a run-in with that font last week, and wasn't able to get a match for it. I went with a similar font that the customer approved.

No idea, but I'd seriously try to talk the customer into something else. That's barely legible (and I'm being generous).
