MaxMetal (really any Polymetal) is a greate substrate when rigidity is not a concern. It is relatively rigid in smaller pieces, however you won't want to use 4'x8' sheets without some type of backing or stringers.
+1 to changing temp settings. On my VJ1304 I turn the pre and platen OFF when printing to banner, and crank my dryer up to 60. (This was based on advice received here)
Also, one thing you way want to do is REALLY give your head a good clean. I had all kinds of little craziness going on there...
Sounds exactly like too much pressure. Also, you don't state whether the print was laminated or not. If it wasn't laminated, then you DEFINITELY need to mask it before application.
Could also be a situation where you used too much application fluid.
The problem with most DTG printers is that they don't print white (some do, but for that kind of money, most people just buy an automated screen press).
Not curing is a big part of the problem, but here is another one to consider - how long are you allowing the alcohol that you are cleaning with to evaporate before you start applying vinyl?

For small "temp" signs (and I define small as 3'x6' or smaller), we dont' laminate and never will unless the client specifically requests it. From an application standpoint, invest in a few sizes of big squeegee, and your unlaminated prints become easier to apply than your laminated ones!

You need to find out if all the vans are stored in the same location - indoor? Outdoor? If indoor, is it a temperature controlled building? This looks like it was parked right up against something that was REALLY hot, for extended exposure.
