They most likely used a Roland, Mimaki, Mutoh or Epson printer. The reason their colors and dot gain look so good is because they make their own profiles.
I don't use Onyx enough to help unfortunately. You will need to use the Mutoh specific crop marks for it to work so if Onyx allows for that then it should work. If onyx doesn't have a Mutoh ValueCut Driver it may just not work at all unless you get a version that does. Hopefully someone else...
It depends on the software you are running but it works the same as Graphtec fundamentally it just uses different crop marks. Let us know what software you use to print and design and we can help a little better.

That's a dirty or damaged encoder strip for sure. Try cleaning it with some isopropyl alcohol and take a nice look to see if there are any damaged spots. If it looks perfectly clean and still gives you this type of print, you probably need to replace it and/or the sensor that reads it.

The ink lines probably just have some air in them from the empty cartridge. A couple strong cleanings should bring it back into the head but if it doesn't it is a sign that your cap tops or pump are not working at 100% efficiency.
