Sometimes this happens to us when we run non-Gerber or "Edge Ready" material. The hole pattern is close, but not quite right. If you haven't changed to an off brand material though, it's probably in line the suggestions above.

I'm guessing that you are printing on Silver metallic? The sensor's eye is red, the mark is black and the material is silver- that color combo will provide very little contrast. if you have a UV printer, print white around the marks. You can also print yellow ink on clear vinyl with a solvent...
