    I would use 1" x 1" standoffs, one in each corner and one centered at the top and one centered at the bottom (6 total). 1/4" acrylic for your backer panel will be fine. The standoffs will keep it from bowing.

    Yeah, I'm usually all for VHB, but save yourself or the next guy some trouble and screw it in. Paint the screws to match the face, if possible.

    There are no codes that I've ever heard of. The only code you're even remotely coming close to is for a sign/object that falls between 27" clearance and 80" clearance, in which case it cannot extend more than 4" from the wall. But you're less than half of that, so that wouldn't affect you.
