My first guess would be that you have a capping station issue. If either the Y or K aren't sealing correctly, the pump will not be able to pull any ink through either of the heads. This will make it not prime, and you can run cleanings until the cows come home with no change at all. When...

Make sure you're using a flashlight when you're cleaning so you can actually see what you're doing. Look very carefully for any lint or fuzzies on the heads and on the wipers. Be sure you're cleaning the wipers and cap tops every week too.

Just screen print 'em. Stouse can knock them out for you quick and cheap. This sounds like a government job.. From my experience, most bids that go out spec a "3M or equivalent", so printing a transparent color over white reflective is perfectly acceptable.

I've seen issues like this when the encoder strip is very dirty. It's usually accompannied by what's called stair-stepping, which is visible on the edges of the print as the print actually moving to one side or the other on the media.
