Drag it into Symbols palette.
Type lettering
Select lettering and type Command/Shift O to create outlines
Command 8 to create compound shape from outlines
I delete color from lettering to better see where I'm going
Create new layer under type and drag symbol to this layer...

The only time I've needed a CNC cut-out I had it cut out first then applied the graphic. Perfect. I know it's not production friendly, but it works.

Make sure the roll pressure was released prior to storage. 4 years will definitely flat spot them. Make sure the rollers are perfect. No nicks, scrapes, gouges etc...
If the rollers are toast the thing is worth scrap rate at the yard.
Job spec calls for white Mylar(printed) with 0.5mil Mylar laminate. Already tried a web search and got nothing. The laminate would have to be cold apply also. Where can I find this stuff? Or is there a MM that can help?

But if you insist on DIY you can CNC the form out of pine and use bondo to finish the imperfections. That and tons of sanding. Forms must be perfect as any flaws will telegraph to the face. Biggest problem would be finding a place that does vac forming...
Our SP540 is off too when cutting laminated prints. Print/Cut the thing can do a couple hundred decals perfectly. Had one if the best techs in the business check it out. Said it was freakishly accurate. But again when in service mode it is a Print/Cut routine. Not print, lam, re-install...
