not sure on your printer but in order to print three layer white you need 2 white print heads that are staggered so that one white head drops down the white ahead of the CMYK heads then the second white head for over coat.
optically clear is a clear smooth vinyl.
matte vinyl is a textured face vinyl so it cant be optically clear because of its nature.
on vehicles you must laminate perf or else you will get water in the holes and visibility will be zero to none and in some places that is illegal.

pricing is usually a local market thing that isnt best done over the internet. A sign shop in downtown Manhattan will have a higher price than for a sign shop in rural Alabama.
call 2 other local sign shops and ask them to quote you the job. then you will see how you fit in.
3641 is not clear it is white and has grey backed adhesive.
3651 is available in clear.

you havent stated what type of printer you have.
oracal 290GF is optically clear and solvent printable.
GF 205 is also solvent printable clear but not optically clear.
paint the board dark red then line them up and run them through a 120" flatbed printer to print the rooster and text on to the boards. then clear coat if needed.

I would have done that truck in 2 horizontal panels. 1 lower section full length and 1 shorter piece for the cab. much faster and no worries about lining up the phone numbers and getting them straight.
