We've had it happen on our 360 a couple times, if it's under warranty you can get a replacement from HP. If it happens often then the ink cartridge sensor is faulty, we had to replace 2.
From what I gathered, once the printer deems a cartridge faulty then it will just keep spitting it out.
We're having the same issues, usually if I click the printer status (blue bubble with exclamation point in the top right) and close it, it will update the media.

Check your media size manager, I believe there are gutter settings in there for all the different media widths. Or you could try sending as a no margin print and it may center better.
I would guess it's the canvas. If it's tight when it leaves your shop then it's stretching out over time, I would check to make sure the canvas is actually meant to be used in stretch frames.
The only thing I could think technique-wise is that maybe it's too tight or your corner brackets are...
Do you take the canvas around to the back of the frame and staple it? We also use a line of glue on the inside edges before we fold up the frames and assemble.
What brand and thickness of canvas are you using?
We do a couple thousand business cards a month in house and it still makes sense to send some of the jobs out to 4Over despite having all the necessary equipment.
Sure you could do the odd specialty thing for fun, but I don't think there's much money in it.
I would convert that PNG to a CMYK jpeg with a white background and make a clipping mask for the circle shape on the placed image instead of placing an RGB PNG with a transparent background. That should also fix your pantone colour because pantones don't get along with transparencies.
Only ever seen that when the files weren't made/saved properly. Is that a CMYK file with a vector pantone swatch and an RGB PNG in it?
Looks like transparency problems that are confusing the RIP or the document colour space is not properly specified.
