I just go by however the vinyl is feeling that day, sometimes it's one way, other days reversing works better. I've never found one way that always works better for everything.

What media are you running? The printer should push the end out instead of bunching up there, might just be a matter of cleaning in the curing unit to reduce friction.

I did a full colour change from black to white on a Ford Dually, the only print was logo on sides and hood, mostly just white unprinted. I would just laminate it.

Those "wings" on the A are too small of details, imagine trying to put that on a shirt. I'm personally not a big fan of those fonts that almost look unfinished where they have curves in strange places that make them look like they were autotraced.
It's possible for them to separate but not likely on a wall graphic. The bond between the lam and print vinyl is much greater than the bond between the vinyl and the wall, so if they shrink at different rates then you'll get the curling at the edges.

That's a generic error, hard to get much info from it. Ink sensor issues seem pretty common on these printers, we had to replace 2 on our 360. Though if all of them are doing it out of nowhere, then I would think it's something to do with the circuit board or possible connector issues. You could...
It should forward out a bunch before it starts printing side A, then you'll want to forward out a couple feet before cutting it off. Roll it up on a core and load it normally. Doing one-offs that small is a pain, you gotta waste much more material than you're actually printing.
Liquid laminate will help with the image. But really it's the installation that's gonna determine the life of the banners. We use the same banner for our pole banners and they need to be installed tight to prevent flapping in the wind. We've had some up for over 3 years now that were installed...
About every 6 months or so, which is probably excessive. I suggest doing it when your SMK3 is at 50% and again when changing the kit out. So every 1-2 years depending on how much you run it. The vent holes are tiny and get clogged easily which affects the curing.
