How long after printing did you apply the white vinyl? If you trap the solvents from the ink by laminating too soon, you can force the solvents to out gas through the clear and adhesive.

This is almost always caused by a poor cap top seal. The printer counts down the ink it uses (or actually thinks it uses) during cleaning cycles. If the seal isn't good (due to poor maintenance, age or defective cap tops, the pumps suck air rather than ink. Once it writes down the chip...

Close Flexi and restart it. It should refresh the media from the printer when you go to the quality tab in Rip and Print. Good Luck

The electrostatic hold down works well. But if you cut into the bed, you'll ruin it and it's not considered to be a replaceable part really. The vacuum table is what we typically sell most often.

Don't rule this out just because you haven't had problems in the past. It appears from the pics you posted that yellow isn't firing as it should on the print on the right. You didn't post a print of the nozzle test - that's always where I start.

No matter how simple your instructions are, you'll eventually run into someone even more simple than the instructions. Sometimes you just can't win.
