Make sure you clean the head mounting area first. Ink tends to build up and sort of glues the head down so it doesn't move easily. Usually that's the case.
The above info is pretty standard. The only thing I might add is, most people don't change dampers until they start having issues. If you replace them once per year along with the rest you don't get any surprise failures. It's not completely necessary but can be useful.

I'd say either the main board or power supply got fried and the fact that it turns on at all probably means the main board. If you can find someone to fix the board that's an option but you probably just need to throw one or both of those boards at it and it should get back up and running.

Sounds like maybe an ink flow issue. I'd probably start by adjusting the positive and negative pressure to make sure it's within range. Also, do you see air coming out during the air purge or is it solid ink?
Error 522 is a media width sensor error. I'd bet the head strike knocked it out of place. Make sure it's sitting in it's spot properly and clean the surface with alcohol. PR errors are similar. There is a sensor in the back of the index head that reads the position of the pinch rollers. If it...

I'm pretty sure this model uses ink that does not have good outdoor durability. I'd stick with Solvent, Latex or UV based printers if you want to do outdoor stuff.
Epson made different versions of the DX5 head that would only work in certain machines. Usually when you put the wrong head in a machine it would print a similar pattern to your test print. I don't think they ever did it with the DX4s though so you issue is more likely the encoder.
Looks like an encoder issue. I would clean the encoder along with the sensor. If that doesn't do the trick, replace them. I've seen that pattern on a locked DX5 head before but your machine uses a DX4 heads and as far as I know they never made locked DX4s.
