We had a seamstress make one for a customer once. It was a sweetener to get the deal. She sewed it from nylon and it turned out well. Just give the seamstress the dimensions and she should be able to figure it out.
Interesting. The only water based ink that I see kill heads fast is Dye sub but the pigment based water inks seem to let the head last forever. It just goes to show how finicky these heads can be. Everyone has a different experience.

I would go into the RIP and check what your rendering intents are set to. Usually the default has CMYK and RGB interpreted differently. I would make the CMYK rendering intent the same as he RGB one and see if it changes for the better.

The quickest and simplest fix would be to re-save the file as a PDF and then in the compression settings make it so any image above 300dpi gets down-sampled to 300dpi. As long as the file is already at a 1:1 ratio, this should have no affect on the output quality but reduce the file size...

I have installed 100's of heads and I never flush before installing. In fact, I have seen more problems from pre-flushing than not. It won't hurt to flush the ink lines first though. You never know what ink was in there and how it settled.

Change the head cables first and clean the terminals that they plug into. You basically have a short in the system and it usually happens in the cable or the terminals. If that doesn't fix it, you probably have a bad head.
Illustrator files are not considered production files and your client just doesn't know what they are talking about. Tell them to save it as an EPS or PDF instead.
PTF is an older way that Flexi used to do profiles. They have a function in the production manager that converts it to an ICC. Check around the menus. I believe it's called, "Convert Legacy Profiles".
